I have been followin wnaysha diet plan for last one month but I don't seem to have much success. I sill eat rice and some coarse wheat (no bread or fermented stuff). I will try this for 1 more month.

This is all nonsense, a regular person doesn't have to do this. Problems lie in the mouth/nasal region and in rare cases Stomach - H Pylori.

Dieting will not help your breath, if anything it will make it smell. That is one of the symptoms of ketosis. And the thread title is hilarious in itself.
Tonsillectomy - Check
Sinus CT Scan - All Clear - Check
Dentist Examination - "Gums very good" - Check
Endoscopy - Check - H Pylori Negative
Post nasal space cyst removed - Check
Wisdom Teeth Extraction - Check
Mouth Swab Clear - Check

Silent reflux/ GERD both can cause halitosis. Diet is the only way to effectively treat reflux long term. Look it up.
