Bad breath - i would look at a vegetarian diet to see if that helps (can take quite a while to see effects) cut out dairy....if you smoke stop, if you don't floss then start. an do it twice a day

Hairy tongue - digestive tract issues maybe? Medications that commonly cause hairy tongue include antibiotics, anti-psychotics, anti-depressants, and anti-cholinergic agents. Other causes include smoking, alcohol, the use of mouthwashes, and exposure to radiation therapy.
Crack on the back of my tongue and a crack form the middle to the front of my tongue - geographic tongue maybe?
Stuffy nose - get a neil med sinus rinse bottle use it twice daily, animal dander or pet fur/hair can effect this
Post nasal drip - again, get a neil med sinus rinse bottle use it twice daily, you could try raising your bed (pillow end) 8 inches, this is ment to help.
Vitamin d deficiency - you are taking supplements now right?

animal dander or pet fur/hair can effect the eyes creating Conjunctivitis which is an inflammation or infection of the clear, protective layer that coats the front part of the eye. Conjunctivitis can be caused by allergies, bacteria, viruses, or toxic substances...you could always use optrex or something to rinse the eyes, you could visit a optician as they have the equipment to check your eyes under magnification an maybe able to spot something.

Depressed - so would i be if i had all that going on, resolve a few things and you may find this starts to improve...also certain trimesters of pregnancy can cause this.
