So what I remember when I was child I got some abscesses on my upper molars on right side, my shitty PND come from right side only and also in that period my bb started.
Osteomyelitis is more common in children because child's growing bones have an increased blood supply and that makes it easier for the bacteria to get into the bone. An infection from nearby soft tissue or a wound may also lead to osteomyelitis. Got this from google.
My theory is that the infection from the bone which is anaerobic bacteria drain in the maxillary sinus mucosa and create the shitty PND.
I think that’s why antibiotics like metronidazole are working temporary, but for osteomyelitis in order to full eradicate the bacteria from the bone the antibiotic therapy should be done for a minimum of 3 months via IV and also like this is hard to cure and the next step is surgery which need to amputate the infected bone.
Regarding diagnosis of osteomyelitis CT are useless, MRI are gold standard but not 100% accurate. Than biopsy of the bone can confirm the disease, but for the biopsy first need to locate the infection via MRI lol
I will appreciate if doctor Murat Aydin could reply about my theory.
Next week I will start to investigate getting MRI etc.
